Twins are genetic, which means you can't force a Sim to have them without using hacks. They're also determined at conception rather than birth. In other words, if a Sim is predisposed to having twins (e.g. one of their parents was a twin or had twins), they're more likely to have twins, but saving right before birth and reloading won't produce twins. If you don't want to wait for the conditions to be right though, Mod The Sims 2 likely has a twin hack or two.
