Can NPCs get pregnant and stay that way? E.g the maid? Or what about your own created sims, would they return home pregnant?

No. The NPC's can't get pregnant, but, with the help of a program by

The NPC Replacer
You can change the way the NPC's look. If you have downloaded some skins from a few that have pregnant skins, you can change the skins of the NPC to make them use the Pregnant skin.
(They have the replacer for Sims 1 and 2)
I use the replacer all the time for different occasions.

Well, err, actually I was talking about TS2. Because I can "woohoo" with Kaylynn Langerak, it's just the "Try For Baby" option doesn't come up. I don't have "risky woohoo" and I don't see how she can get pregnant without this option.
Edit: Yeah, OK, fine, the InSIMenator, but apart from that? Would I have to move her in / take away her NPCyness?

NPCs cannot get pregnant, unfortunately, because they do not have a home where they can raise the baby. If you move an NPC (like Kaylynn) into an existing character's house, then she will be able to get pregnant if you "Try For Baby" with a male Sim. Also, if your female Sim does "Try For Baby" with a male NPC, then she will become pregnant with that male NPC as the father, or if a female Sim comes over to male Sim's house and they "Try For Baby", then the female will go home and become pregnant, even if she has a husband and children already.

A female sim can get pregnant on her own lot (with anyone) but if a woman goes to a man's house, he can't get her pregnant at his house, unless they live together.
