I think you have to go in a cott or get your parent sim to go to the wardrobe and click "Change Toddlers Clothing"

You need to buy the dipper changer thing in the missalashion section then get a sim to select it and it should say plan everyday for... (Toddlers name) and plan PJ's for... (Toddlers name). It only says that if you have some clothes the toddler can change into.
